Hygia for Providers
Extend the care relationship beyond the exam room.
A patient may spend 20 minutes with a physician and thousands of hours managing their health outside the practice. That is when the care plan meets real life.
Did the patient understand what the physician said? Did they start the medication? Schedule the referral? Complete the test? Did a new question arise? Did something change?
Physicians and their teams cannot personally remain in contact with every patient between every visit. Hygia helps them stay connected anyway.
The challenge
The patient leaves. The responsibility for the care plan does not.
The traditional healthcare encounter has a beginning and an end. The patient's healthcare journey does not.
A physician may diagnose a condition, explain a treatment plan, prescribe medication, order testing, make a referral and provide clear instructions. Then the patient goes home.
From that point forward, the provider often has limited visibility into what happens until the patient returns, calls the office or another event appears in the medical record. That creates an enormous blind spot: the provider knows what was recommended, but may not know what happened afterward.
What Hygia does
Hygia extends the physician relationship. It does not replace it.
Through Gia, patients have a familiar point of contact they can return to over time. Gia can follow up after visits, reinforce the care plan, answer questions about next steps, prepare patients for appointments and help them follow through on referrals and testing.
Gia is not a physician, does not diagnose and does not make clinical decisions. Where clinical judgment is required, the appropriate member of the care team becomes involved — with the relevant context already assembled.
The goal is not more messages into the practice. It is fewer avoidable ones, and better-informed human attention where it is warranted.
Use cases
Where practices use it.
These are not disconnected programmes. They can occur through one persistent relationship.
- Pre-visit preparation
- Post-visit follow-up
- Care-plan reinforcement
- Medication questions
- Appointment reminders
- Referral follow-through
- Preventive outreach
- Patient education
- Chronic-care engagement
- Emerging-concern identification
- Staff escalation
- Patient experience
The value
What changes.
Continuity
The relationship with the patient persists between encounters rather than restarting at each visit.
Reach
Stay connected with more patients than a staff-only follow-up model can support.
Understanding
Know whether the plan was understood and acted on, not only what was recommended.
Workflow support
Flagged information routes to the right person with context, rather than arriving as undifferentiated inbox volume.
Focused human attention
Scarce clinical time is spent on the patients and situations where it matters most.
Patient experience
Patients have somewhere to turn when healthcare becomes confusing — and it still leads back to your team.
The same five steps
Connect → Engage → Understand → Act → Learn.
Every audience meets the same relationship cycle from a different side of it.
- 1Connect
Hygia connects the patient to the organizations and people supporting their healthcare journey.
- 2Engage
Gia maintains an ongoing, natural conversation rather than relying solely on episodic outreach.
- 3Understand
Over time, those interactions can reveal what the person understands, intends to do, is worried about and what may be preventing them from moving forward.
- 4Act
Hygia can help translate that understanding into the appropriate next step, whether that means helping the patient, supporting follow-through, creating a task or bringing the right person into the loop.
- 5Learn
Each interaction adds context to the relationship, allowing future engagement to become more relevant and informed.
Across the entire cycle
Memory and persistent context — the relationship does not restart at each interaction.
The visit ends. Your responsibility for the plan does not.
Hygia keeps the relationship alive between encounters and brings your team in when it should be your team.
